Case Study – Shortness of Breath – Adult Female
A 63-year-old woman undergoes total knee replacement for severe degenerative joint disease. Three days after surgery, she develops an acute onset of shortness of breath and right-sided pleuritic chest pain. She is now in moderate respiratory distress with a respiratory rate of 30/min, heart rate of 122 bpm, and blood pressure of 112/74 mm Hg. Oxygen saturation is 89% on room air. Lung examination is normal. Cardiac examination reveals tachycardia but is otherwise unremarkable. The right lower extremity is post-surgical, healing well with 1+ pitting edema, calf tenderness, erythema, and warmth; the left leg is normal. She has a positive Homan sign on the right.
