In your Research Plan you mentioned that you will be conducting a survey using a questionnaire to collect information to do your research.

Prepare a questionnaire that you will use to collect data for your research from your target group.
IMPORTANT: Don’t change the topic of research and send a questionnaire based on a new topic. It must be to the topic on which you wrote your Research Proposal

Guidelines to follow when doing the Assignment
Please read carefully and understand the instructions before doing the work
The questionnaire should have a good heading which will give sufficient information and confidence to the respondent to fill it for you. See the sample below. Write something like this but make sure that you don’t copy this student’s topic title also (by mistake). This is a past student’s work with the student’s permission.

Dear Respondent,
This questionnaire is as a part of the course work for the ESL programme which I am currently taking. The information given is strictly for the purposes of research and will therefore be treated with confidentiality

There should be a minimum of 25 main questions in total in the whole questionnaire.
Count a closed question and its follow up open question that come after it as 1 unit. (That means a question answering “Yes or No” and then its explanation should be counted as 1 item) .
